Fill in an ICO complaint form

If you’ve been unable to resolve your data protection complaint yourself, here's where you can make a complaint to us about an organisation's use of your personal information - for example if they haven't allowed you to see it or you want to complain about how they are handling or processing it.

To help us to look at your complaint as quickly as we can we will need:

  • some basic information, including your name, address and which organisation you’re complaining about
  • details of your data protection problem, and how you want things put right
  • copies of the letters you have sent to and received from the organisation
  • details such as the policy number or account number that your complaint relates to.

We will use the information you provide to help us contact the organisation. If we don’t have enough information we will need to ask you for it, which will mean that it’ll take us longer to look into your complaint.
