[DATE]
Dear Sir/Madam
I wrote to you on [date] to request some information.
[Delete this sentence if you've not received a response] You acknowledged my request on [date] and provided the reference number: [Insert if provided].
A copy of my request [and your acknowledgement] is attached.
I understand that a public authority has 20 working days to comply with information requests and you have now exceeded the statutory timescale. I would therefore be grateful if you could provide your response as soon as possible.
If a response is not received by the close of business on [date + 10 working days], I intend to complain to the Information Commissioner and request assistance in this matter.
Yours faithfully
